A baseline survey is the starting point for effective community work. It helps KDSSS identify priority needs, plan the right support, and measure change over time—so resources reach the families who need them most.

Household profile

Basic household details, living conditions, and key challenges to understand the context of each family.


Education & child wellbeing

School attendance, learning needs, child protection concerns, and access to nutrition and care.


Health & access to services

Health awareness, access to basic care, disability or chronic conditions, and barriers to treatment.


Livelihoods & women empowerment

Skills, income sources, employment stability, self-help group participation, and opportunities for training and support.

Who conducts the baseline survey?

KDSSS field staff and trained community volunteers conduct surveys through household visits and community meetings.

Is the survey mandatory?

No. Participation is voluntary. Families can choose not to answer any question or stop at any time.

How is my information used?

Information is used to understand needs, plan services, and measure program results. We do not sell personal data.

Will my information be kept confidential?

We handle information responsibly and share only aggregated results for reporting whenever possible.

Do I receive a submission reference ID?

Yes. After submission, you’ll see a confirmation message with a reference ID in this format: KDSSS-YYYY-SerialNo-HouseholdID (serial number resets each year).

How can I submit the survey?

You can submit through KDSSS staff during field visits, or contact us to arrange a survey appointment for your area.
